Outpatient Consultation
Assessment: Metastatic Melanoma.
Recommendations: I had a long discussion with the patient and her husband regarding the nature of her illness and its poor responsivity to any systemic therapy. We talked about interleukin-2 and other treatments, but she readily told me that she was not interested in pursuing these. Quite frankly, I do not blame her giving their low response rate and high toxicity profile. We also discussed the possibility of doing an axillary excision of the adenopathy. I told her that this probably would not make her live any longer but would improve her quality of life with less of a chance of pain and swelling of the arm which would be important as she is right-handed. Of course, there would be no point in doing this operation if the PET scan showed extensive metastasis elsewhere. We talked about doing a CAT scan of the brain, since this is a famous place that melanoma likes to go, but once again, she declined this. I got the sense that she may decline all surgeries and all therapies and rely on her religious convictions for the duration of her life. I told her the latter may be as soon as within the 6-12 months. She seemed understanding of this.

Regardless of the grim prognosis, we believed that our great God had and still has much more work for both of us to do before He welcomes us home to be with Him.
